Referrals are accepted from optometrists, physicians, and registered nurses.

If you are a healthcare professional and would like to refer, please complete and fax our referral form below:

NOTE: Do not rely on fax submissions for URGENT referrals. Please call our clinic and we will do our best to accommodate your request. Alternatively, patients can present to the closest emergency room.
